IMPORTANT PLEASE READ!!!!!
If your child gets a new physical completed after April 15, 2019, the physical will be good for the 2019-2020 school year and current spring tryouts and future practices.

The Tennessee Secondary School Athletic Association (TSSAA), by law, requires that all students have a physical and concussion form completed by a licensed physician before the student can tryout for any athletic sport each academic year. All parents and students must sign a concussion's form to be submitted to the school where they plan to try out in an athletic sport. Download the forms below or retrieve one in person at the Crestview Middle School front office. All physicals and concussion forms must be completed before your child can try out.

You will need Adobe Acrobat to read and print the document.
