The Tennessee Secondary School Athletic Association (TSSAA), by law, requires that all students have a physical and concussion form completed by a licensed physician before the student can tryout for any athletic sport each academic year. All parents and students must sign a concussion's form to be submitted to the school where they plan to try out in an athletic sport. Download the forms below or retrieve one in person at the Crestview Middle School front office. All physicals and concussion forms must be completed before your child can try out.

****It is THE LAW that any student interested in trying out for a sport, MUST HAVE a TSSAA physical and concussion form, signed by a licensed physician, on file here at Crestview Middle. No physical, no tryout.
 
Physicals must be renewed annually in order to participate in sports. 
 
Packets may be picked up during regular school hours between 7:00am and 4:00pm Monday through Friday, except holidays and no-class breaks. You may also download copies of the forms above below the Adobe Reader logo.
Currently limited accessibility to the office during breaks.
